These Terms & Conditions (“Terms”) govern the use of transportation services provided by Carolyn Family Services, LLC (“Company,” “we,” “our,” or “us”). By booking or using our services, you agree to the following terms:
1. Services Provided
Carolyn Family Services, LLC offers private pay Non-Emergency Medical Transportation (NEMT) for clients who need safe, reliable transportation to and from medical and related appointments.
2. Booking & Payment
All rides must be booked in advance by phone, email, or through our approved booking process.
Payment is due at the time of booking or upon service completion, unless otherwise agreed in writing.

3. Cancellations & No-Shows
Cancellations should be made at least 24 hours in advance.
Late cancellations or no-shows may result in a fee.
Repeated no-shows may result in refusal of future service.

Seatbelts must be worn at all times.
All vehicles are strictly smoke-free. Absolutely no smoking, vaping, or use of tobacco, marijuana, or any other substance is permitted inside our vehicles at any time.

Carolyn Family Services, LLC takes every reasonable step to ensure safe and reliable service. However, we are not liable for missed appointments, indirect costs, or damages arising from delays, cancellations, or circumstances beyond our control.
7. Conduct Policy
Respectful behavior is expected from all clients and staff.
Abusive, unsafe, unlawful, or intoxicated behavior may result in immediate termination of service and possible reporting to authorities.
